CESS-129. Cognitive/Behavioral Treatment of Tobacco Dependence in a Medical Setting

Learning Objectives: Identify the six stages of the trans-theoretical model of the stages of change. Describe three of the five requisite skills of Motivational Interviewing. Explain the five elements included in the structure of a consultation.

Abstract: The Mayo Clinic Nicotine Dependence Center (NDC) in Rochester, MN, has 14 years of experience in the treatment of nicotine dependence in a medical setting. Treatment interventions for more then 24,000 patients have been conducted in inpatient and outpatient settings. In addition an 8-day residential (inpatient) treatment program is offered for the more severely dependent. The patients have the advantage of an integrated approach based on three programs within NDC (Research, Education, and Treatment). In this workshop a clinician counselor from the Treatment Program will use case examples and discussion/interaction with the learners to describe the structure of a patient intervention. The five elements of the consultation (build rapport/trust, assess stage of change, menu of alternative behaviors, increase readiness for change/remove barriers, and support for maintaining abstinence) will be examined. The benefits of using the Stage of Change model (pre-contemplation, contemplation, preparation, action, maintenance, and relapse) of Prochaska and Di Clemente, as well as the approach of Motivational Interviewing of Miller and Rollnick, within the framework of the 45-60 minute consultation will be discussed. Learner participation through interactive examples will illustrate key principles of these models of patient interaction.
