Achieving System-wide Success with the Clinical Practice Guideline

Learning Objectives: Demonstrate how a major Midwest health system used patient intervention as recommended in the Clinical Practice Guideline: Treating Tobacco Use and Dependence to improve the quality of health care.

Abstract: Audience: Health care professionals; clinicians; coalitions advocating cessation treatment in a clinical setting;tobacco control advocates

Key Points: Presentation outlines the step-by-step process that Aurora Health Care used to implement the Clinical Practice Guideline: Treating Tobacco Use and Dependence throughout its system. Through a successful pilot program in one of the organization’s regions, Aurora Health Care was able to demonstrate that intervening with its tobacco-using patients improved the quality of health care in that region, and throughout its system.

1) Audience will understand the challenges that faced Aurora Health Care as it prepared to implement the Clinical Practice Guideline: Treating Tobacco Use and Dependence. Many of the challenges facing healthcare organizations have universal qualities. Audience may recognize Aurora’s challenges as their own.

2) Audience will understand how Aurora Health Care met those challenges by building organizational momentum with education, self-assessment and partnering with natural allies.

3) Measuring the impact of Clinical Practice Guideline: Treating Tobacco Use and Dependence is a challenging and continual process. Audience will discover ways Aurora Health Care measured and validated success.

Educational Experience:Presenter will use lecture, PowerPoint, videotape and interaction with audience to reinforce teachable moments of this presentation.

Benefits: By learning about the experience of a health system with more than 600 providers, the audience will better understand how to successfully implement and measure the Clinical Practice Guideline: Treating Tobacco Use and Dependence.
