Learning Objectives: List ways single nicotine patch sales & prescription protocols boost access to treatment

Abstract:
Problem/Objective: Access to tobacco dependence treatment is often limited by cost and lack of insurance coverage. Nicotine patches are sold only in one or two week supplies, with a higher initial cost than tobacco. We sought to lower the cost barrier by selling single nicotine patches, and establishing a protocol for dispensing of prescription nicotine medications (inhaler, nasal spray) and bupropion (Zyban/Wellbutrin) by trained pharmacists.
Methods: We began a pilot project of selling single nicotine patches at a participating pharmacy, by prescription, to lower the initial cost of the patch. We have also begun a prescription protocol involving the pharmacy and a prescribing physician, to make prescription nicotine medications and bupropion available without the cost barrier of a physician office visit.
Results: With availability of single nicotine patches for $3.50 and prescriptive authority for nicotine medications and bupropion, the cost barrier has been lowered, allowing tobacco users to obtain tobacco dependence treatment at a local pharmacy at lower cost.
Conclusions: Sale of single nicotine patches should be made available more widely, and prescription protocols instituted, where allowed by state pharmacy guidelines. Reducing cost barriers improves access to tobacco dependence treatment.
