Integrating Statewide Quit Lines into Managed Care Organizations

Abstract:
Problem/Objective:Progress has been made to increase rates of clinicians asking smoking status and advising patients to quit, research suggests that there continue to be low rates of arranging follow-up for smokers. UW-CTRI outreach program goals are to ensure that healthcare providers in Wisconsin use the 5As (ask, advise, assess, assist and arrange) for all smokers willing to make a quit attempt. Establishing a Quit Line Fax-Back program increased referrals to the Quit Line and the likelihood that providers routinely achieve the fifth A -- arranging follow-up. It also proved an effective strategy to engage managed-care organizations in cessation activities.
Methods:UW-CTRI has established partnerships with all 26 managed care organizations in Wisconsin and after a successful pilot established fax-back referral programs at 50 clinic sites. Data is collected through program monitoring.
Results:The pilot study with a large managed care organization was successful in increasing the number of patients enrolled in cessation treatment as well as low burden to all involved. The number of patients contacted by the Quit Line as a result of the fax referral will be reported monthly beginning May 2003 and be shared at the session.
Conclusions:Establishing fax-back quit line referral programs at managed care organizations can meet the needs of healthcare providers seeking evidence-based programs, and the patient who will receive a proactive call from the Quit Line upon receipt of the fax-referral form and is an effective method for integrating quit lines into managed care and engaging them in cessation support activities.
